Every person is different, so the level of support that each person needs differs. We provide individualized support for whatever a member may need, from attaining safe housing, to finding a job, to budgeting for groceries. Our case managers maintain the clubhouse philosophy that individuals are the key to guiding their own treatment.
To qualify for case management, the potential recipient must be a member of Breakthrough Club and must qualify for Medicaid assistance. This is the only area of the club where Medicaid is required.
