Community Support
This aspect of Breakthrough provides individualized and intensive services for members who have a variety of needs. More importantly, case managers create relationships that help nurture and guide members through difficult life experiences, which are often attributed to the symptoms of their mental illness. Community support services assist the members in becoming and remaining independent.
